I have read, understand and agree to this Application Form, the Standard Form of Agreement (SFOA), the Critical Information Summaries (CIS) pertaining to my chosen services and other information provided with this Application form and our Schedule of Fees and Charges. I understand that by transferring my services to innoTel, this may result in finalisation of my account
with my existing service provider. I also understand that this may trigger early termination fees with
my existing service provider and it is my responsibility to check any current contract and early
termination obligations with my existing service provider.

I understand that my first month’s bill may be higher than the sum of the monthly fees outlined in this application due to part month charging (pro-rated charging) and any once-off / up-front fees. For more information see our guide on pro-rated billing.

I understand that the nbntm broadband service I am purchasing is not a guaranteed speed service
and that any speeds quoted are in reference to the maximum possible speed a service can achieve
(i.e. up-to the quoted speed). The speed and throughput of a broadband service is determined by a
number of factors, including but not limited to, condition of the infrastructure, remote sites where data
is being retrieved from and other factors, some of which are out of innoTel’s control.

I understand that a site visit from an nbncotm sub-contractor is required to install the service and
associated equipment. This includes gaining access to the inside and outside of the premise and an
authorised person over the age of 18 must be present for the duration of the installation. I also
understand that innoTel has no control over date or timing of the installation and that a complex
installation may require an additional visit(s) to complete the work

I understand that innoTel does not provide battery backup for nbntm services and I also understand
that during a mains power outage, my internet connection and any telephone services (VoIP) that
rely on this internet connection will not work and I won’t be able to make calls, including to
Emergency 000 services. I understand that if I use a telephone service that relies on the internet that
my mobile phone should be fully charged in the event I need to make emergency calls

I understand that, where applicable, by accepting this agreement I am waiving my rights and protections provided under the Telecommunications (Customer Service Guarantee) Standard (the "CSG"). For further information, see our CSG waiver.

I understand that if any equipment is provided as part of this application, the warranty for that equipment extends to the life of the contract or 12-months, whichever term is longer

I understand that if I cancel these services before the end of the minimum term, an early termination
fee is applicable. The early termination fee calculation is: Monthly Fee x Remaining Months = Early
Termination Fee.

I understand that this Application to innoTel for services does not attract a cooling off period and I also understand that if I withdraw this Application and Order after submitting to and being accepted by innoTel, a Withdrawal fee of $100.00 (Inc. GST) per service will be applicable.

I understand that innoTel will collect information about my business, including but not limited to contact details and other information necessary to provide me with services. innoTel’s Privacy Policy describes how we use this information.
